We are a primary care clinic and our focus is on caring for our patients as a whole person, not just a set of symptoms.

Samira is a doctor of nursing and specialized as a family nurse practitioner. She is a firm believer in preventative care and goes above and beyond to assure her primary care patients are well screened, cared for, supported, and educated to reach or stay at optimal health. Her philosophy of care is a biology-based, holistic approach to health and wellbeing.

We do NOT provide ongoing pain management. If you require pain management, we can provide you primary care support and refer you to a pain clinic for pain management.

Primary CareWhat is ADHD?

Attention-deficit/hyperactivity disorder is a chronic condition which refers to impulsiveness, attention difficulty, and hyperactivity. It doesn’t have anything to do with how smart a person is.

What are the signs?

When you have ADHD, you may experience some of the following:

“I have an appointment in 3 hours so I don’t have time to do anything before.”

“I have difficulty staying tuned. I drift off easily but I can refocus after a few minutes.”

“I try to learn different skills pretty frequently but I stop trying after the first few challenges.”

“I listen to the same song repeatedly and every time I hear something new.”

“I get extremely excited about things and completely forget about them after a few minutes.”

“If I get bored, I will leave the room no matter what is going on.”

“I randomly worry about something that may happen in the future.”

“I have the instant urge to clean up my room/house and I may stop and leave after an hour or get busy doing something else.”

What happens if I leave my ADHD untreated?

ADHD may result in or worsen low self esteem, complicate or trouble your relationships, and make it difficult to advance at work or school.

What treatments are available?

Two major treatments are medication and behavior therapy. We provide medication management.

What are the causes of ADHD?

The causes are mainly unknown, but some research connects it to genetics.

What is the process at your clinic?

Needless to say, diagnosis of ADHD is a complex process.

The first step is to schedule an initial visit. If you are an established patient, telehealth is OK. We will perform a comprehensive evaluation to assure the diagnosis is appropriate. We will review your lifestyle, challenges, and strengths. There will always be a consultation regarding non-pharmacologic approaches to improve your quality of life and medication management as needed.

We only refill ADHD medications (stimulants) for 18+ patients. Monthly follow ups are required to make sure safe and appropriate prescription.

What We Know About ADHD Overdiagnosis

A physical exam should be completed annually. During the annual physical our provider checks your overall health.

This exam is about 30 minutes, which includes evaluating the basic function of your organs, reviewing your vaccinations, and helping you get healthy or maintain good health.

Our blood testing is a comprehensive review of your health status and we often receive praise from our patients for the extent of screening that we provide. We also provide hormone testing and reviews which help our patients to better understand their health status.

Primary CareSex is an important part of our lives. We will support you with whatever lifestyle you have. Our services include confidential STD testing, education, preventative care, emergency contraception, and same-day STD prescription.

If you have multiple sex partners or a partner that has multiple partners, you have a new partner, you have been having genital pain, new rashes, discomfort during urination, or bleeding, you should take advantage of our confidential STD testing services.

STDs are harmful and important to diagnose and treat as soon as possible. We will test you in the clinic and provide you with same-day treatment.

What is STD?

STD stands for sexually transmitted disease. An infection transmitted through sexual contact, caused by bacteria, viruses, or parasites. These pathogens may live in your body for a long time and if not treated properly, will affect your immune system and your overall well-being.

What are some examples?

  • HPV infection
  • Genital herpes
  • Chlamydia
  • Gonorrhea
  • HIV/AIDS
  • Syphilis
  • BV, bacterial vaginosis is not an STD but can increase your chance of getting infected with STDs.

Primary CareThe safety of our drivers is important to us. If your duties are deemed safety-sensitive by the Department of Transportation (DOT), we will provide you same-day appointments to assure you receive a proper physical exam and be able to drive a commercial vehicle safely. What is a DOT physical? A DOT physical refers to the physical examination that assures you are capable of maintaining your CDL (commercial driver’s license). Your physical check up needs to be performed every 24 months or sooner depending on your health status and special conditions you may have.

What should I bring to my appointment?

  • Valid government-issued identification – for example, driver’s license or passport
  • The list of your current medications
  • The list of all your other providers, especially your cardiologist
  • Your eyeglasses or contact lenses if you use them
  • Your hearing aids if you use them

What is being done at my appointment?

We will:

  • Test your vision and hearing
  • Check your blood pressure and heart rate
  • Complete a urine test to check for diabetes and other possible conditions
  • Conduct a full physical exam

If all goes well, we will issue your DOT certificate so you can renew or obtain your license.

Medical Examination Report Form – This form is an example of what patients can expect. Our provider will complete this form at the appointment.

Medical Examiner’s Certificate – This form is an example of what certified drivers can expect. Certified drivers will receive their certificate after the exam is completed.

Insulin-treated Diabetes Mellitus Assessment Form – Commercial drivers with insulin-treated diabetes need to have this form completed by their treating clinician no more than 45 days prior to examination by a certified medical examiner (CME). This form is required before a Medical Examiner’s Certificate Form can be issued to a certified driver.

Dr. Samira is a firm believer in looking at a person as a whole and not a set of symptoms. She intends to take a careful approach in diagnosis and treatment of mood disorders. She believes that many mood disorders stem from other diseases or deficiencies in your body and rushing into medication management sometimes only helps to mask those symptoms without actually treating the underlying cause.

Your first mental health visit will be composed of a comprehensive analysis, history taking, and standardized detail gathering which will yield to her treatment plan.

The treatment plan may include all or a few of the following:

  • Referral for behavioral therapy
  • Referral to psychiatry

Primary CareTraveling around the world is a great experience which exposes you to different cultures and activities. It is fun and educational. You can spend an amazing time bonding with your loved ones through travel, meeting new people, and learning about life. International trips also come with some risks regarding your health. There are vaccination requirements specific to many countries which will boost your immune system. Our pre-travel consultation includes complete evaluation and planning for your whole family. We will review your country of destination, your immunization history, your general health, and status of any chronic conditions you may have. Our comprehensive travel consultation visit also includes:

  • Advice for pregnant travelers
  • Child travelers
  • Prophylactic antibiotics and respiratory and infectious treatment
  • Vaccination consult (not administration)
  • Providing a list of items required to take with you including insect repellent, sunscreen, small first-aid kit, any medications you take in their original containers, and common items such as acetaminophen, ibuprofen, and first aid creams, which can be challenging to buy when traveling.

Annual well-woman exam
Annual check up is one of the most important components of women’s health and essential to monitor the changes your body is going through from puberty to menopause.

Pap smear (cervical cancer screening)
Cervical cancer screening is provided and takes less than 10 minutes. Schedule an appointment to collect a sample and get tested.

Family planning (consultation and prescription for birth control)
Birth control, also known as contraception, is a device, medicine, or activity that prevents pregnancy by stopping sperm from fertilizing an egg. There are many different types of birth control, including:

  • Barrier methods: Condoms, contraceptive sponges, and female and male condoms
  • Hormonal methods: Pills, patches, rings, injections, arm implants, and IUDs
  • Surgical methods: Vasectomies and tubal ligations

We provide you a safe place to share your goals and concerns and we will help you to choose the best birth control option that matches your goals and your lifestyle.

Sexually transmitted diseases or infections
We provide same-day testing and treatment. Our STD services are confidential.

Chaperones
The American College of Obstetricians and Gynecologists (ACOG) recommends that a chaperone be present for all pelvic exams, as well as other genital, breast, and rectal exams. A chaperone can be a trained medical professional or other person who acts as an impartial observer. Their role is to:

  • Be sensitive and respectful of the patient’s dignity and confidentiality
  • Be aware of signs of distress or discomfort
  • Know how to raise concerns if they have them about the medical professional’s behavior

Before the exam, the healthcare provider should explain what will happen and offer the patient the option to have a chaperone. Patients can decline a chaperone if they are adults or 12 years or older and can make their own medical decisions. For children 8 or younger, a parent or guardian can act as the chaperone.

We do not have a chaperone available to offer to our patients but we welcome and encourage them to bring a chaperone with them to the appointment. If you choose to have the procedure done by yourself, that is OK and acceptable.
